IFPCS InterPig DataBase Entry Form

Instructions for completing the entry form

Please complete and submit a separate form for each distinct item to be entered into the IFPCS DataBase.

Please complete all the blanks below and submit your entry by pushing the "Submit Form" entry button. Not all web browsers will support forms. If you are not using Netscape, I recommend that you get a copy by following this link. If you cannot use a web browser that supports forms, please e-mail Vince Hearing at hearingv@nih.gov, and an e-mail form will be sent to you for your submissions.

After submission, you should receive a reply via e-mail that your information has been received. If you do not receive such notification within one week, please contact Bill Oetting via e-mail at this link or by Phone at (612) 624-1139 or FAX at (612) 624-6645.

Sample Information

Item - {e.g. InterPig Administrator probe} Enter the specific name of the reagent, antibody, probe, etc.

Description - {e.g. Reagent which electronically detects InterPig Administrators} Enter a brief description of the material.

Category - {e.g. Other} List the most relevant category for the material. Please chose from one of the following categories:

Animals
Biochemistry
Cell Biology
Cell Culture
Chemistry
Dermatology
Electron Microscopy
Immunology
Melanoma
Molecular Biology
Physics
Other (please specify in the Comments section)

Quantity - {e.g. 5 kg} Indicate the quantity of the reagent supplied in a single request.

Reference - {e.g. R.K.Brodinsnag, JIR, (1997)} List the relevant citation for the reagent (if any).

Comment - {e.g. Tests with this probe have so far detected three cases} List any further comment on the reagent.

Contact Information - please provide complete contact information.

Sample Information: Please fill out the entire form. For selection of the 'category', click and hold on the rectangular box labeled 'Animals' to see the complete menu of choices and select the most appropriate category by dragging to that item and releasing. Use the TAB key to move from item to item for entry.

Contact Information: Please provide complete contact information. Be sure to include your e-mail address, if you have one. Source information in the database will include a hyper-link to your e-mail address allowing individuals to contact you directly.
